Report:  Patient ID Card

This report is used to generate patient ID cards, to different paper or card stock depending on your printer setup/preference configuration.  The name, address and contact information of the Home Clinic is presented, along with standard identification information for the patient, including SSN, Patient ID, Date of Birth, Date of Admission, a patient photo, and a physical description.  Other text appearing on the card can be customized on the Preferences screen, in Label Preferences - ID Cards.  Patient ID cards can also be printed with a barcode, for retrieving patients in the system using a barcode scanner or allowing patients to check in at a self-check-in kiosk.

Report Field Explanations

(Home Clinic)

This field displays the 'Clinic Name' of the Home Clinic as entered on the Clinic Information screen.

(Home Clinic Address/Phone)

This field displays the 'Street Address', 'Street Address (2)', 'City', 'State', 'Zip', and 'Phone' number of the Home Clinic as entered on the Clinic Information screen.

(Patient Photo)

This field displays the patient's photo, as saved for each on the Patient Profile screen.

Name

This field displays the full name of the patient as entered on the Patient Profile screen.

SSN

This field displays the 'SSN' number entered for the patient on the Patient Profile screen.

ID

This field displays the 'Patient ID' assigned to the patient on either the Patient Profile or Patient ID Administration screens.

DOB

This field displays the 'Birth Date' entered for the patient on the Patient Profile screen.

DOA

This field displays the most recent of either the 'Intake Date' or 'Latest re-admission date' entered for the patient on the Dosing Information screen.

Hair

This field displays the 'Hair Color' selected for the patient on the Patient Profile screen.

Eyes

This field displays the 'Eye Color' selected for the patient on the Patient Profile screen.

Hgt/Wgt

This field displays the 'Height' and 'Weight' values/text entered for the patient on the Patient Profile screen.

Physical Description

This field displays the 'Other Descriptive Information' text entered for the patient on the Patient Profile screen.

(First/Second/Third Message)

Text may or may not appear on Patient ID cards in these fields at your facility.  Additional text can be added to appear in these fields on the Preferences screen, in Label Preferences - ID Cards.

Doctor's Name

This field displays the 'Doctor' entered for the Home Clinic on the Clinic Information screen.

Patient Signature

This signature line is for the patient's signature, for validating the Patient ID card.

Patient ID

This parameter allows users to limit report data to only print a Patient ID card for the patient whose 'Patient ID' is entered here.
