Report:  Treatment Plan Templates

This report displays the content of treatment plan templates entered on the Treatment Plan Templates screen.  This report appears much like the Treatment Plans and Treatment Plan Summary reports, but displays entered templates which do not include any patient information.  This report is generally rarely used to review the treatment plan templates currently available for selection when adding a new treatment plan on the Treatment Plans screen.

Report Field Explanations

Tx Plan Name

This field displays the 'Template Name' for each template as entered on the Treatment Plan Templates screen.

Created By

This field displays the User ID of the user who created the template in Methasoft.

Created On

This field displays the date the template was created in Methasoft.

Problem

This field displays each Problem's 'Problem Text' as entered on the Treatment Plan Templates screen.

Added

This field displays the date each Problem was added to the template on the Treatment Plan Templates screen.

Ind./Cat.

This field displays each Problem's 'Index/Category' as selected on the Treatment Plan Templates screen.

Status

This field displays each Problem's 'Status' as selected on the Treatment Plan Templates screen.

Severity

This field displays each Problem's 'Severity Rating' as selected on the Treatment Plan Templates screen.

Goal

This field displays each Goal's 'Goal Text' as entered on the Treatment Plan Templates screen.

Term

This field displays each Goal's 'Term' as selected on the Treatment Plan Templates screen.

Duration

This field displays each Goal's 'Duration (Days)' as selected on the Treatment Plan Templates screen.

Target Date

This field is irrelevant on this report.

Next Review Date

This field is irrelevant on this report.

Method

This field displays each Method's 'Method Text' as entered on the Treatment Plan Templates screen.

Method (Date)

This field is irrelevant on this report.

(Plan Item Type) Intervention

This field displays each Intervention's 'Intervention Text' as entered on the Treatment Plan Templates screen.
