This report displays a list of all active patients in the facility including basic dosing information for each patient, in the form of a medication sign out sheet, with spaces for dispensing personnel to manually document what medication is administered to each patient. Some facilities use this sheet daily to have a manual backup record of dispensing activity, which can be very useful if errors are made resulting in inventory discrepancies between the physical and computer inventory counts. It is also used to ensure dispensing staff are double-checking all medication being administered to patients. Other facilities choose not to use this report based on concerns regarding patient confidentiality and/or the ability of patients to see other patients' dosage amounts. Some facilities choose to use Methasoft's electronic signature pad functionality as an alternate to using this report so that patients can sign for medication received.
Patient ID
This column displays the 'Patient ID' number entered for each patient on the Patient Profile screen.
Drug Type
This column displays the 'Drug Type' selected for each patient on the Dosing Information screen.
Dose Type
This column displays the 'Dose Type' selected for each patient on the Dosing Information screen.
Last Dose Taken On
This column displays the last 'Take On' date for medication previously received by each patient, viewable on the Dosing History screen.
Current Dose
This column displays the total daily dosage amount in mgs entered for each patient on the Dosing Information screen.
Mgs Observed
This column is intended for dispensing personnel to manually document the total number of mgs ingested by each patient at the dosing window.
# of T/Os
This column is intended for dispensing personnel to manually document the total number of takeout doses administered to each patient.
Total Mgs
This column is intended for dispensing personnel to manually document the total number of mgs of medication administered to each patient, including observed and takeout doses.
Total # of Doses
This column is intended for dispensing personnel to manually document the total number of daily doses administered to each patient.
Dispensed By
This column is intended for dispensing personnel to manually initial or sign each medication transaction on this report. Some facilities also have the patient initial this column beside the dispensing person's initials, others have patients initial just to the right of this column.
Total
This field displays the total number of patients displayed on this report.
