Report:  SNAP

This report displays all SNAP profile entries entered on the SNAP screen over a selected date range.  Each patient's SNAP history is displayed on it's own page, allowing this report to be printed for large date ranges then filed in individual patient charts.  All data entered on the SNAP screen is displayed on this report for each SNAP entry.  This report is generally printed periodically and filed in patient charts, and/or reviewed during patient treatment plan reviews.

Report Field Explanations

Patient Name

This field displays the 'Patient ID' number and full name of the patient for whom the SNAP entries were entered.

SNAP Date

This field displays the 'SNAP Date' selected on the SNAP screen for each SNAP entry.

Entered By

This field displays the 'Employee ID' (User ID) of the user who entered each profile on the SNAP screen.

Strengths

This field displays the 'Strengths' text entered for each SNAP entry on the SNAP screen.

Needs

This field displays the 'Needs' text entered for each SNAP entry on the SNAP screen.

Abilities

This field displays the 'Abilities' text entered for each SNAP entry on the SNAP screen.

Preferences

This field displays the 'Preferences' text entered for each SNAP entry on the SNAP screen.

Criteria Explanations

Patient ID

This parameter allows users to limit the report's data to only display SNAP profiles recorded for the patient whose 'Patient ID' is entered here.

From Date

This parameter allows users to limit the report's data to only display SNAP profiles entered with a 'SNAP Date' on or after the date selected here.

To Date

This parameter allows users to limit the report's data to only display SNAP profiles entered with a 'SNAP Date' on or before the date selected here.

Entered By

This parameter allows users to limit the report's data to only display SNAP profiles entered by the user selected here.
