Report:  Referral List - Treatment Event

This report displays a list of all patient referrals entered on the Make Referral screen over a selected date range, and indicates which treatment event led to each referral.  Each patient's Referral Date is displayed with the treatment event leading to each referral.  Totals are provided for the total number of referrals and the number made for each treatment event.  This report is generally used to view a more detailed breakdown of referrals by treatment event than is displayed on the Referral Summary report.

Report Column Explanations

Patient ID

This column displays each referred patient's 'Patient ID' as assigned to each on the Patient Profile screen.

Patient Name

This column displays each referred patient's full name as entered for each on the Patient Profile screen.

Referral Date

This column displays the 'Referral Date' entered for each referral on the Make Referral screen.

Intake

This column displays 'Yes' if 'Intake' was selected in the 'Referral is part of' field on the Make Referral screen.  'No' appears in this column if a different treatment event was selected.

Psychosocial

This column displays 'Yes' if 'Psychosocial' was selected in the 'Referral is part of' field on the Make Referral screen.  'No' appears in this column if a different treatment event was selected.

Treatment Plan

This column displays 'Yes' if 'Treatment Plan' was selected in the 'Referral is part of' field on the Make Referral screen.  'No' appears in this column if a different treatment event was selected.

Other

This column displays 'Yes' if 'Other' was selected in the 'Referral is part of' field on the Make Referral screen.  'No' appears in this column if a different treatment event was selected.

Report Field Explanations

Totals

This field displays the total number of referrals with a 'Referral Date' included in the selected criteria date range, as well as the total number of 'Yes' values for each of the treatment event columns.

Criteria Explanations

From Date

This parameter allows users to limit report data by displaying only patient referrals with a 'Referral Date' on or after the date selected here.

To Date

This parameter allows users to limit report data by displaying only patient referrals with a 'Referral Date' on or before the date selected here.
