This report displays a breakdown by number and percentage of patient referrals entered on the Make Referral screen, broken down by referral type, patient phase level, and treatment event for each quarter of the previous year from a selected criteria 'From Date'. A breakdown of referrals by whether or not 'Contact Information' was provided is also included, along with a breakdown of referrals entered for people on the Waiting List. This report is generally used with the Referral Summary report to review patient referral activity and/or provided to governing/auditing agencies.
Past 3 Months
This column displays patient referral totals data for the last 3 months from the 'From Date' criteria selection made when this report is generated. Breakdown totals are displayed by 'Number' and 'Percentage'.
Past 3-6 Months
This column displays patient referral totals data for the last 3-6 month quarter from the 'From Date' criteria selection made when this report is generated. Breakdown totals are displayed by 'Number' and 'Percentage'.
Past 6-9 Months
This column displays patient referral totals data for the last 6-9 month quarter from the 'From Date' criteria selection made when this report is generated. Breakdown totals are displayed by 'Number' and 'Percentage'.
Past 9-12 Months
This column displays patient referral totals data for the last 9-12 month quarter from the 'From Date' criteria selection made when this report is generated. Breakdown totals are displayed by 'Number' and 'Percentage'.
Referrals by Category
This section displays the total number of patient referrals broken down by the 'Referral Type(s)' selected for each on the Make Referral screen, for each quarter of the past year from the criteria 'From Date' selected when this report is generated.
Referrals by Phase
This section displays the total number of patient referrals broken down by referred patient 'Phase Type', selected for each on the Dosing Information screen, for each quarter of the past year from the criteria 'From Date' selected when this report is generated.
Referrals by Treatment Event
This section displays the total number of patient referrals broken down by the 'Referral is part of' option selected for each on the Make Referral screen, for each quarter of the past year from the criteria 'From Date' selected when this report is generated.
Patient given Referral Contact Information
This section displays a breakdown of referral totals by the number of referrals entered with the 'Contact Information Provided' field checked on the Make Referral screen, for each quarter of the past year from the criteria 'From Date' selected when this report is generated.
Referrals from Waiting List
This section displays the number and percentage of patient referrals made for people on the waiting list, which is determined by the number of people on the Waiting List changed to a 'Status' of 'Referred to another facility' on the Waiting List screen within the past year from the selected criteria 'From Date'. This breakdown is presented for each quarter of the past year from the criteria 'From Date' selected when this report is generated.
From Date
This parameter allows users to define the year of data to be displayed by selecting this date. If today's date is selected, then referral data will be presented for each quarter of the past year.
