Report:  Referral - Contact Information

This report is similar to the Referral Agency List report and displays all referral agencies entered on the Referral Agencies screen, including the name, address, phone and fax numbers, and contact person for each.  Additional information is also displayed for each agency.  This report is generally used for reference and provided to patients as a source of referral information.  This report is also generated when the Print button is used on the Make Referral screen, in which case it will only display referral agency information for the selected referral.

Report Field Explanations

Facility Name

This column displays the 'Name' of each facility as entered on the Referral Agencies screen.

Address

This column displays the 'Address' of each facility as entered on the Referral Agencies screen.

City, State, Zip

This column displays the 'City', 'State' and 'Zip' of each facility as entered on the Referral Agencies screen.

Additional Information

This field displays the 'Additional Information' text entered for each agency on the Referral Agencies screen.

Phone

This column displays the 'Phone' number of each facility as entered on the Referral Agencies screen.

Fax

This column displays the 'Fax' number of each facility as entered on the Referral Agencies screen.

Contact

This column displays the name of the 'Contact' person for each facility as entered on the Referral Agencies screen.
