This report displays all patient referrals entered on the Make Referral screen over a selected date, including detail information for each referral, such as the 'Referral Agency' each patient was referred to, the date of each referral, as well as the treatment event which led to each referral. This report is generally printed periodically in facilities documenting patient referrals and filed in each patient's chart as needed.
Patient ID
This column displays the 'Patient ID' number of each patient referred to an outside agency.
Patient Name
This column displays the full name of each patient referred to an outside agency, in 'Last Name'/'First Name' format.
Referral Agency
This column displays the 'Referral Agency' selected for each referral on the Make Referral screen.
Referral Date
This column displays the 'Referral Date' entered for each referral on the Make Referral screen.
Intake
An 'X' will appear in this column for each referral where 'Intake' was selected in the 'Referral is part of' field on the Make Referral screen. This column will appear blank if a different treatment event option was selected.
Psychosocial
An 'X' will appear in this column for each referral where 'Psychosocial' was selected in the 'Referral is part of' field on the Make Referral screen. This column will appear blank if a different treatment event option was selected.
Treatment Plan
An 'X' will appear in this column for each referral where 'Treatment Plan' was selected in the 'Referral is part of' field on the Make Referral screen. This column will appear blank if a different treatment event option was selected.
Others
An 'X' will appear in this column for each referral where 'Other' was selected in the 'Referral is part of' field on the Make Referral screen. This column will appear blank if a different treatment event option was selected.
Given Contact Info.
This column displays 'Yes' for each referral where the 'Contact Information Provided' field was checked on the Make Referral screen. 'No' is displayed for each referral without a checkmark in this field.
Modified By
This column displays the 'Employee ID' (User ID) of the user who entered each referral in the system.
Total Referrals
This field displays the total number of patient referrals displayed on the report.
Patient ID
This parameter allows users to limit the report's data to only display referrals entered for the patient whose 'Patient ID' is entered here.
From Date
This parameter allows users to limit the report's data to only display patient referrals with a 'Referral Date' on or after the date selected here.
To Date
This parameter allows users to limit the report's data to only display patient referrals with a 'Referral Date' on or before the date selected here.
Referred By
This parameter allows users to limit report data by displaying only those patient referrals entered by the user selected here. Available selections include the full names of all active users.
Referral Agency
This parameter allows users to limit report data by displaying only those patient referrals where the patient was referred to the agency selected here. Available selections include agencies entered on the Referral Agencies screen.
Site
This parameter is irrelevant for Methasoft users. Only a facility's Home Site will appear available for selection in this field, and 'All' will be selected by default.
