Report:  Patient List with SSN

This report displays a list of all active patients in the system with basic patient information for each, including each's social security number.  This report is printed in some facilities as a reference for useful patient information.

Report Column Explanations

Patient ID

This column displays the 'Patient ID' number of each patient as entered on the Patient Profile screen.

CID

This column displays the 'State ID (CID)' number of each patient as entered on the Patient Profile screen.

Insurance ID

This column displays the 'Insurance ID' number of each patient as entered on the Patient Profile screen.

Patient Name

This column displays the full name of each patient as entered on the Patient Profile screen.

SSN

This column displays the 'SSN' number entered for each patient on the Patient Profile screen.

Counselor

This column displays the 'Employee ID' (User ID) of the 'Counselor' assigned to each patient on the Patient Profile screen.

Report Field Explanations

Total Active Patients

This field displays the total number of patients displayed on this report, which is the total number of active patients attending your facility.

Criteria Explanations

Counselor ID

This parameter allows users to limit the report's data to display only patients assigned on the Patient Profile screen to the 'Counselor' selected here.

Site

This parameter is irrelevant for Methasoft users.  Only a facility's Home Site will appear available for selection in this field, and 'All' will be selected by default.
