Report:  Patient List with Last Drug Screen

This report displays a list of all active patients with each patient's last drug screen test result.  Each patient's ID, name, phase level, attendance schedule, and assigned counselor are also included.  Each patient's last drug screen test result's test date, positive substances, and additional comments are displayed.  This report can also be run to only display unacceptable drug screen test results by checking the 'Unacceptable Only' criteria checkbox.  This report includes a total for the number of patients appearing on it.

Report Column Explanations

Patient ID

This column displays each patient's 'Patient ID', as entered on the Patient Profile screen.

Patient Name

This column displays each patient's full name, as entered on the Patient Profile screen.

Phase

This column displays each patient's 'Phase Type', as selected on the Dosing Information screen.

Dosing Schedule

This column displays an abbreviated version of each patient's 'Schedule Type' and 'Weekly Schedule', as selected on the Dosing Information screen.

Last Drug Screen Test

This column displays the 'Test Date' of each patient's last drug screen test result.

Last Drug Screen Results (Positive Results Only)

This column displays abbreviations for all substances which tested positive on each patient's last drug screen test result.

Last Drug Screen Results Other

This column displays any 'Other Information' entered for each patient's last drug screen test result.

Counselor

This column displays each patient's assigned 'Counselor', as selected on the Patient Profile screen.

Report Field Explanations

Total Patients

This field displays the total number of patients appearing on this report.

Criteria Explanations

Phase

This parameter allows users to limit the report's data by displaying only patients on the 'Phase Type' selected here, as stored for each patient on the Dosing Information screen.

Unacceptable Only

This parameter allows users to limit the report's data by displaying only patients whose last drug screen test result tested positive for one or more substances considered to be illicit, or unacceptable, at your facility.

Counselor

This parameter allows users to limit the report's data by displaying only patients assigned on the Patient Profile screen to the 'Counselor' selected here.
