Report:  Patient List by Counselor

This report displays all patients in the facility, including basic patient information and sorted by counselor, as dictated by 'Counselor' assignments made on the Patient Profile screen.  This report is generally only used to review the quantity of patients on each counselor's caseload, or is sometimes printed by counseling staff to use as a check-off sheet for a case management task.  There are numerous 'Patient List' reports in the system that display different data for each patient and/or are sorted differently.

Report Column Explanations

CID

This column displays each patient's 'State ID (CID)' number as recorded on the Patient Profile screen.

Sex

This column displays each patient's 'Gender' as recorded on the Patient Profile screen.

Active

This column displays whether or not each patient is 'Active', as recorded on the Patient Profile screen.  'Yes' indicates the 'Active' field is checked on the Patient Profile screen.

Home Clinic

This column displays each patient's 'Home Clinic' as recorded on the Patient Profile screen.

Intake Date

This column displays each patient's 'Intake Date' as recorded on the Dosing Information screen.

Report Field Explanations

Counselor

This field displays the name of the counselor assigned to the list of patients displayed below.

ID

This field displays the user ID of the counselor assigned to the list of patients displayed below.

Counselor's Total Patients

This field displays a total count of the patients currently assigned to the counselor on the Patient Profile screen.

Criteria Explanations

Counselor ID

This parameter allows users to limit the report data to just one selected counselor's caseload.  Available selections include all users with a 'Counselor' job role on the Employee Administration screen.

Patient ID

This parameter allows users to limit the report's data to only display the patient whose 'Patient ID' is entered here.

Include Inactive Patient

This report displays only 'Active' patients by default.  This parameter allows users to expand the report to also display all inactive patients.

Site Group

This parameter allows users to limit report data by displaying only data for patients who are members of the Site Group(s) selected here, as assigned to each patient on the Patient Profile screen.

Site

This parameter is irrelevant for Methasoft users.  Only a facility's Home Site will appear available for selection in this field, and 'All' will be selected by default.
