Report:  Patient List by City / State

This report displays a list of all patients in the system, including the 'City' and 'State' of each 'Patient Address' for each patient.  This report is most commonly used for an overall review of where a facility's patients are attending from, often when considering where another treatment facility is located or needed.  There are numerous 'Patient List' reports in the system that display different data for each patient and/or are sorted differently.

Report Column Explanations

Patient ID

This column displays each patient's 'Patient ID' number.

Patient Name

This column displays each patient's full name, in 'Last Name'/'First Name' format.

CID

This column displays each patient's 'State ID (CID)' number.

City

This column displays the text entered in the 'City' field on the Patient Addresses window for each patient's 'Patient' address.

State

This column displays 'State' selected on the Patient Addresses window for each patient's 'Patient' address.

Report Field Explanations

Total Patients

This column displays the total number of patients displayed on the report.

Criteria Explanations

Counselor

This parameter allows users to limit the report data to display only patients assigned a particular 'Counselor' on the Patient Profile screen.

Site Group

This parameter allows users to limit report data by displaying only patients who are members of the 'Site Group(s)' selected in this field, as assigned on the Patient Profile screen.

Site

This parameter is irrelevant for Methasoft users.  Only a facility's Home Site will appear available for selection in this field, and 'All' will be selected by default.
