Report:  Patient List

This report displays a list of all patients in the system, including general profile information for each patient such as each's treatment 'Modality', 'Employment' status, 'Marital Status', 'Race', 'Gender', 'Patient Status' and assigned 'Counselor', as entered for each on the Patient Profile screen.  This report is generally only used to review an active list of patients in the system.  There are numerous 'Patient List' reports in the system that display different data for each patient and/or are sorted differently.

Report Column Explanations

Patient ID

This column displays each patient's 'Patient ID' number.

Patient Name

This column displays each patient's full name, in 'Last Name'/'First Name' format.

CID

This column displays each patient's 'State ID (CID)' number.

Modality

This column displays the treatment 'Modality' selected for each patient on the Patient Profile screen.

Employment Status

This column displays the 'Employment' status selected for each patient on the Patient Profile screen.

Marital Status

This column displays the 'Marital Status' selected for each patient on the Patient Profile screen.

Gender

This column displays the 'Gender' selected for each patient on the Patient Profile screen.

Race

This column displays the 'Race' selected for each patient on the Patient Profile screen.

Status

This column displays the 'Patient Status' selected for each patient on the Patient Profile screen.

Counselor

This column displays the 'Counselor' assigned to each patient on the Patient Profile screen.

Report Field Explanations

Total Patients

This field displays the total number of patients displayed on this report.

Criteria Explanations

Modality

This parameter allows users to limit the report data to display only patients with a particular 'Modality' selected on the Patient Profile screen.

Employment Status

This parameter allows users to limit the report data to display only patients with a particular 'Employment' status selected on the Patient Profile screen.

Status

This parameter allows users to limit the report data to display only patients with a particular 'Patient Status' selected on the Patient Profile screen.

Gender

This parameter allows users to limit the report data to display only patients with a particular 'Gender' selected on the Patient Profile screen.

Include Inactive

This checkbox determines whether or not inactive patients will be included on this report.

Marital Status

This parameter allows users to limit the report data to display only patients with a particular 'Marital Status' selected on the Patient Profile screen.

Race

This parameter allows users to limit the report data to display only patients with a particular 'Race' selected on the Patient Profile screen.

Counselor

This parameter allows users to limit the report data to display only patients assigned a particular 'Counselor' on the Patient Profile screen.

Site Group

This parameter allows users to limit report data by displaying only patients who are members of the 'Site Group(s)' selected in this field, as assigned on the Patient Profile screen.
