Report:  Intakes and Discharges by Treatment Episode

This report displays a list of all patient intakes and discharges over a selected date range, but based only on treatment episodes entered for each patient on the Treatment Episodes screen.  The 'Episode Start Date' entered for each patient serves as the 'Intake Date' for each episode.  The 'Episode End Date' entered for each patient serves as the 'Discharge Date' for each episode.  Totals are provided for the total number of Intakes and Discharges.  This report can be very useful in facilities tracking treatment episodes for each patient, to provide intake and discharge information and totals.

Report Column Explanations

Patient ID

This column displays the 'Patient ID' number of each patient as entered on the Patient Profile screen.

Patient Name

This column displays the full name of each patient as entered on the Patient Profile screen.

CID

This column displays the 'State ID (CID)' number of each patient as entered on the Patient Profile screen.

Modality

This column displays the 'Modality' selected for each patient on the Patient Profile screen.

Intake Date

This column displays the 'Episode Start Date' entered for each episode for each patient on the Treatment Episodes screen.

Discharge Date

This column displays the 'Episode End Date' entered for each episode for each patient on the Treatment Episodes screen.

Modified By

This column displays the 'Employee ID' (User ID) of the user who entered each patient's treatment episode.

Report Field Explanations

Intakes/Discharges

These fields serve as headers, dividing this report into two sections, one displaying all patient 'Intakes' and the other displaying all patient 'Discharges'.

Additional Info

This field displays the 'Additional Information' text entered for each treatment episode on the Treatment Episodes screen, and only appears if the 'Show Additional Info' checkbox is checked when running this report.

Total Intakes/Total Discharges

These fields follow each of the two sections of this report, and display the total number of 'Intakes' and 'Discharges' for the selected date range.

Criteria Explanations

From Date

This parameter allows users to limit the report's data to display only treatment episodes with an entered 'Episode Start Date' or 'Episode End Date' on or after the date selected here.

To Date

This parameter allows users to limit the report's data to display only treatment episodes with an entered 'Episode Start Date' or 'Episode End Date' on or before the date selected here.

Show Additional Info

This parameter allows users to expand the report's data to also display the 'Additional Information' text entered for each treatment episode on the Treatment Episodes screen.
