This report displays all patient health screen results as entered on the Health Screen screen, including the date and results of each screening and 'Other Test' results. This report is generally printed periodically, or as needed for patients then filed in patient charts. Some facilities only print this report for patients with specific health conditions that require more intense monitoring of these health measures. Some facilities are required to collect this data periodically for all or certain subsets of patients, in which case this report is sometimes generated for auditing purposes. The 'Specific Test' criteria field allows users to run this report to check for 'All', 'Last', or 'Missing' TB and Hepatitis C test results for a selected date range and/or patient, to help facilities ensure that mandatory tests are being administered and recorded in Methasoft within mandatory time frames.
Screen Date
This column displays the 'Screening Date' selected on the Health Screen screen for each result recorded.
HIV Results
This column displays the 'HIV Test Results' selection made on the Health Screen screen for each result.
TB Results
This column displays the 'TB Test Results' selection made on the Health Screen screen for each result.
HCV Results
This column displays the 'Hep C Results' (Hepatitis C) selection made on the Health Screen screen for each result.
Pregnancy Results
This column displays the 'Pregnancy Results' selection made on the Health Screen screen for each result.
BPM (Pulse Rate)
This column displays the number entered in the 'Pulse Rate (BPM)' field on the Health Screen screen for each result.
RPM (Respiratory Rate)
This column displays the number entered in the 'Respiratory (RPM)' field on the Health Screen screen for each result.
BP (Blood Pressure)
This column displays the numbers entered in the 'Blood Pressure' fields on the Health Screen screen for each result.
Ht (Height)
This column displays the Height recorded in total Inches, which is calculated based on the entries in the 'ft.' and 'in.' Height fields on the Health Screen screen for each result.
Wt (Weight)
This column displays the number entered in the 'Weight' field on the Health Screen screen for each result, which is the patient's weight as measured in pounds.
BMI (Body Mass Index)
This column displays the calculated Body Mass Index (BMI) value displayed in the 'BMI' field on the Health Screen screen for each result.
Additional Info
This column displays the text entered in the 'Additional Comments' field on the Health Screen screen for each result.
Test Type
This column only appears if 'Other Results' were recorded on the Health Screen screen, and displays the 'Test Type' selected for each additional test on the Health Screen Test Add/Edit window.
Min Value
This column only appears if 'Other Results' were recorded on the Health Screen screen, and displays the 'Minimum Value' for the additional test selected on the Health Screen Test Add/Edit window, as defined in the 'Health Screen Test Types' code table.
Max Value
This column only appears if 'Other Results' were recorded on the Health Screen screen, and displays the 'Maximum Value' for the additional test selected on the Health Screen Test Add/Edit window, as defined in the 'Health Screen Test Types' code table.
Result
This column only appears if 'Other Results' were recorded on the Health Screen screen, and displays the number entered in the 'Test Value' field for each additional test on the Health Screen Test Add/Edit window.
Out of Range
This column only appears if 'Other Results' were recorded on the Health Screen screen, and displays an 'X' for each additional test 'Result' entered on the Health Screen Test Add/Edit window that falls out of range for the selected 'Test Type'. If a 'Test Value' entered for a selected 'Test Type' falls outside of the range of the test's 'Min' and 'Max' values, an 'X' will appear in this column for the result.
Patient ID
This field displays the 'Patient ID' number for the patient whose Health Screen results are displayed below.
Other Tests
This field serves as a header for 'Other Results' and does not appear on this report unless 'Other Results' are recorded on the Health Screen screen.
Total Health Screens
This field displays the total number of Health Screen entries recorded for the patient over the selected date range.
Patient ID
This parameter allows users to limit report data by displaying only one patient's health screen results over the date range selected. If this field is left blank, all health screen results will be displayed for the selected date range, with each patient's results appearing on a new page.
From Date
This parameter allows users to limit the report's data by displaying only health screen results with a 'Screening Date' on or after the date selected here.
To Date
This parameter allows users to limit the report's data by displaying only health screen results with a 'Screening Date' on or before the date selected here.
Specific Test
This parameter allows users to limit the report's data by not only displaying health screen results for TB and Hepatitis C tests, but also by using 'All', 'Last' or 'Missing' options to display all, missing, or the last of each of these test types for the selected date range and/or patient criteria. This criteria field can be very useful for ensuring that mandatory tests are being administered and recorded according to mandatory time frames.
