Report:  Central Registry (Texas)

This report displays a list of all active patients including Central Registry information required for facilities in the state of Texas.  Each patient's intake or latest re-admission date, last initial, date of birth, eye color, gender, race, last 4 SSN digits, and Patient ID is displayed, along with the total number of patients.

Report Column Explanations

Admit Date

This column displays the most recent of the 'Intake Date' or 'Latest Re-admission date' for each patient, as entered on the Dosing Information screen.

Patient Initial

This column displays the last initial for each patient, pulled from the 'Last Name' field on the Patient Profile screen.

Birth Date

This column displays each patient's 'Birth Date' as entered on the Patient Profile screen.

Eye Color

This column displays a database eye color value for each patient, based on each patient's 'Eye Color' as selected on the Patient Profile screen.

Gender

This column displays a database gender value for each patient, based on each patient's 'Gender' as selected on the Patient Profile screen.

Race

This column displays a database race value for each patient, based on each patient's 'Race' as selected on the Patient Profile screen.

SSN

This column displays the 'SSN' entered for each patient on the Patient Profile screen.

Patient ID

This column displays the 'Patient ID' assigned to each patient on either the Patient Profile or Patient ID Administration screens.

This field displays the total number of patients included on this report.
