This report displays all patient assessments entered over a selected date range, including those that were entered and not marked as complete. Available criteria filters allow users to display only assessments that were entered but not marked as complete, or only patients for which an assessment was not entered. The type of assessment entered and the dates on which an assessment was entered and/or completed are included on this report. This report is generally used by administrative or counseling staff to ensure needed assessments have been completed.
Assessment Type
This column displays the name of each assessment entered, incomplete, or missing, as defined on the Assessment Wizard screen.
Patient ID
This column displays the Patient ID of each patient as entered on the Profile screen.
Patient Name
This column displays the full name of each patient as entered on the Profile screen.
Start Date
This column displays the date on which each assessment was initially saved in the system.
Complete Date
This column displays the date on which each assessment was marked as complete in the system.
From
This parameter allows users to limit the report's data by only displaying assessment audit data for assessments entered on or after the date selected here.
To
This parameter allows users to limit the report's data by only displaying assessment audit data for assessments entered on or before the date selected here.
Assessment Type
This parameter allows users to limit the report's data by only displaying assessment audit data for the specific assessment selected here.
Show Incomplete Only
This parameter allows users to limit the report's data by only displaying assessments that were saved but never marked as complete on the Patient Assessments screen.
No Assessment Entered
This parameter allows users to limit the report's data by only displaying patients for whom assessments have not been entered.
